premium based on the categories you select from the drop down choices of: employer type, coverage selected, age and level of annual gross compensation.
Note:
Grandfathered rates apply only to employees who had supplemental coverage and met the age requirements on April 1, 2008. For any new hires or employees obtaining supplemental coverage through Evidence of Insurability, the Grandfathered rates are not applicable.
